Health & Immunization Records

Applicants will be required to provide standard health and immunization records as required by Tennessee law and campus safety policies. This information is collected solely for the purpose of:

  • Ensuring compliance with state health regulations

  • Protecting the safety and well-being of all students, staff, and faculty

  • Identifying any accommodations or health supports that may be needed during enrollment

Important: Health information will not be used to deny admission on the basis of disability or medical condition. Records will be kept confidential in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and the Family Educational Rights and Privacy Act (FERPA).

Admissions Interview

All applicants will participate in an admissions interview to help LTDU staff understand their goals, strengths, and support needs. The interview is a collaborative conversation, not a test, and is designed to ensure that Living the Dream University is the right fit for the student.

Applicants may choose to complete the interview:

  • In Person – on LTDU’s campus with staff, and if desired, with a family member, guardian, or support person present.

  • Virtual – using secure video conferencing (Zoom, Google Meet, or similar), allowing participation from home or another safe environment.

During the interview, LTDU staff will:

  • Learn about the student’s educational and career goals

  • Discuss areas where the student may need additional support

  • Answer questions about courses, campus life, housing, and services

  • Provide families/support persons the opportunity to ask about accommodations and resources

Important: The interview is not used to screen out applicants. Instead, it helps create a Personal Success Plan tailored to each student’s needs.
